- Complete our application online OR print this PDF and submit in person or via mail.
- Contact our Admissions Nurse via phone at 906-483-4317 or via email at ttolonen@jsneuro.com.
- Ensure that all insurance policies are active and up to date.
- After the application is submitted, your name will be added to our inquiry list.
- Please be patient while you are on the inquiry list.
- Once you are approaching the top of the inquiry list, you will be contacted by the Admissions Nurse to discuss the next steps to make the transition.
- Once accepted, you will need a routine physical with your primary care physician. Your forms from the visit will be sent to the Canal View Admissions Nurse to ensure eligibility requirements are met.
Qualifying Hospital Visit
- Notify the hospital Discharge Planner that your preference is Canal View. The Discharge Planner will contact the Canal View Admissions Nurse to start the admissions process.
- Your hospital Discharge Planner will make a referral to Canal View for consideration. Your Discharge Planner is responsible for ensuring that all information in the referral packet is complete and accurate. Delays or inaccurate information from the hospital setting will result in the possible lack of a bed at the facility.
- After your complete referral packet is submitted by your hospital Discharge Planner, Canal View will verify your insurance and eligibility.
- If you are eligible, Canal View will inform your hospital Discharge Planner and they will discuss the next steps towards your transition.
*A qualifying hospital stay is a three-night minimum inpatient hospital stay.
